--Ice machine observed with encrusted debris on non-food-contact surfaces.

--HVAC system located above shelving in kitchen, observed rusted with condensation, can potentially contaminate food-contact equipment.

--Back exit door located in the storage area of the food facility has a gap and does not protect against the entry of insects, rodents, and other animals.

--Floor under shelving in kitchen area was dirty and observed with food debris.

--Food facility inspection indicates evidence of rodent activity under shelf areas, but facility does have a pest control program.

--Cleaners, taken from bulk supplies, were not marked with the common name of the chemical (corrected on site). 

--Raw chicken was stored above ready-to-eat food held in the walk-in refrigerator (corrected on site). 

--Refuse is not being stored in receptacles or waste handling units that are inaccessible to insects and rodents.
